Immunization Requirements
For Students who attend school in the Diocese of Dallas

 Required Immunizations

Age Group

Required for entry to school, age or grade, TDH requirements

Required Number of Doses

 DTP/DtaP

Diptheria-Tetanus-Pertussis

Diptheria-Tetanus-acellular Pertussis

Diptheria-Tetanus (DT)

Tetanus-Diptheria (TD)

DtaP will satisfy all DT requirements at all ages

Ages 3 and 4 years

4 doses

 

Entry into kindergarten

5 doses. The fifth dose is not required if the fourth dose in the series was given on or after the 4th birthday. (The calendar month prior to the 4th birthday is no longer acceptable, grades 1-12 grand fathered in.)

 7 years and older

3 doses including 1 dose on or after 4th birthday and 1 dose within the past 10 years. Pertussis not required.

Polio

Any combination of oral polio vaccine (OPV) and inactivated polio vaccine (IPV) acceptable

 Ages 3 and 4 years

 3 doses

 Upon entry into Kindergarten

 4 doses are required, one or which is required on or after the 4th birthday. Or, if the third dose was administered on or after the 4th birthday only 3 doses are required. (The calendar month prior to the 4th birthday is no longer acceptable, grades 1-12 grand fathered in.) Any combination of four doses of OPV/IPV before age 4 is acceptable and a booster is not required.

 18 years and older, not required

 18 years and older, not required

 HibCV

 15 months thru 4 years

Proof of HibCV for children 5 years and older is not required

1 dose on or after 15 months OR a primary series (2 doses, 2 months apart) and a booster (at least 2 months after the last dose) was completed prior to or at 15 months of age

 Measles (Rubeola)  (2 doses)

 PK-3, PK-4, K (1st dose)

 (Required 1st-12th grade)

1 dose on or after the 1st birthday, if received on or after 9/1/90. OR if received prior to 9/1/90 and during the calendar month of the 1st birthday is acceptable OR serologic proof

Entry in Kindergarten  (2nd dose)

(Required 1st-12th grade)

 2nd dose required by entry into Kindergarten

 Rubella (German Measles)

Entry into PK-3, PK-4 or K

(Required 1st-12th grade)

1 dose on or after the 1st birthday OR serologic proof of rubella immunity

 Mumps

Entry into PK-3, PK-4, K

(Required 1st-12th grade)

1 dose on or after the 1st birthday OR serologic proof of mumps immunity

 Hepatitis B

 PK-3, PK-4, K, 1, 2, 3, 4, 5,

and 7, 8, 9, 10

 Three doses of hepatitis B vaccine are required

  Varicella (Chicken Pox)

 PK-3, PK-4, K, 1, 2, 3, 4 and 7, 8, 9, 10

One dose (1) varicella received on or after the first birthday is required. Or written verification of disease from parent, dr. or nurse required in child’s file.

Two (2) doses are required if the child was thirteen (13) years old or older at the time of the first dose of varicella. Or written verification of disease from parent, dr. or nurse required in child’s file.

  Hepatitis A

PK-3, PK-4, K, 1, 2, 3

Required for children who reside in Grayson county are required.

  Two doses (2) are required

1.        Birth is not acceptable for recording 1st dose of Hepatitis B, since 9-1-92, required to have month/day/year.

2.        Receipt of the dose up to (and including) 4 days before the birthday will satisfy school entry immunization requirement.
