| | If your child needs to have a prescription medication in the clinic, please
read the guidelines.
If your child needs to have an over-the-counter medication in the clinic for
more than 14 days, you will need to submit a note from the prescribing doctor
along with the medication
authorization form. Please make sure the note is legible and has the
student's name on it. A note written on a prescription pad is okay.
If your child will need the medication at school for less than 14 days, you only
need to complete a medication
authorization form. The medication will be sent home after 14 days.
Please carefully consider what medications you send to school. Medications that
are most often used by the students are acetominophen, ibuprofen, and inhalers.
Some medications that are stored in the clinic have never been given.
